I know that the San Antonio Abad Hotel in Miraflores (on Ramon Ribeyro) is nice. It doesn't have that many rooms but they are comfortable and equipped with bath, TV, phone and security box. They can pick you up at the airport if you make a fax reservation. Fares vary between 46-65$.
